NHS Cheshire and Merseyside and Wellbeing Enterprises CIC shortlisted for the 2025 HSJ Partnership Awards

NHS Cheshire and Merseyside (Halton Place) and Wellbeing Enterprises CIC are delighted to announce that We Connect Social Prescribing has been shortlisted for Primary and Community Care Project of the Year at the HSJ Partnership Awards, recognising an outstanding dedication to improving healthcare and effective collaboration with the NHS.
Now in their eighth year, the HSJ Partnership Awards, have become the most recognised and respected mark of the strongest relationships between suppliers and the NHS.
Following the thorough judging process, NHS Cheshire and Merseyside and Wellbeing Enterprises CIC were among the 180+ organisations, projects and individuals that made the final shortlist amid 280+ entries logged for the 2025 awards programme.
We Connect is a holistic social prescribing service integrated with all GP practices in Halton which has supported thousands of patients in the last 12 months. The service is evidencing a range of positive patient health/wellbeing outcomes, reducing demand on stretched primary care services and improving patient flow. The service adopts a preventative and early-intervention approach by tackling the root causes of poor health, building strengths and capabilities and connecting patients to community health assets and resources.

The selected winners will be announced during the awards ceremony at Evolution London on 20 March 2025.
