Please note: no providers on this list are currently able to prescribe GLP 1 weight loss medications semaglutide (Wegovy) or tirzepatide (Mounjaro). Plans to support this are currently being developed and will be communicated during the first half of 2025.

The criteria is the same across Cheshire for Tier 3 provision:

  • BMI between 35kg/m2 and 40 kg/m2 and an obesity-related condition that may be improved following weight loss (for example, diabetes, high blood pressure, sleep apnoea/obesity hypoventilation syndrome)
  • BMI of between 40kg/m2 – 49kg/m2
  • BMI of more than 50 kg/m2
  • Adults requiring bariatric surgery who fulfil one of the referral criteria above, and can provide / commit to the requirements below:
  • NHS England requires evidence of weight loss activity prior to referral to Tier 3 services
  • Willing to engage with Tier 3 weight   management service for required duration
  • Willing to attend at least 80% of appointments
  • Willing to engage with specialist service including dietitian, medical, psychology, activity specialist
  • RECENT height, weight and BMI included in the referral form
Halton

Healthy Weight Programme

Fresh Start Weight Management Programme - Halton Borough Council

Specialist Weight Management Service is integrated into this programme

Criteria for tier 3:

  • Client must be 18 or over 
  • Live, work or have a GP in Halton Borough
  • Have a Body Mass Index (BMI) 50kg/m2
  • Have a BMI 45kg/m2 with 2 or more co-morbid conditions*
  • Client is requesting bariatric surgery or use of medication for weight loss
  • Client has answered positively to binge eating questions when triaged by PSW– must also have a BMI 35kg/m2 
  • Client with BMI 30-34.9kg/m2 who has been diagnosed with
  • Type 2 diabetes within the past 10 years who wants to consider surgery
  • Client has been unsuccessful with weight loss in Tier 2 (by 3 months no loss or weight increasing)

(Adjust BMI by 2.5 kg/m2 for people of South Asian, Chinese, Other Asian, Middle Eastern, Black African or African Caribbean family background as these groups are prone to central adiposity and their cardiometabolic risk occurs at a lower BMI.)

  • The service is for adults aged 18+ living or registered with a GP in Knowsley, with a BMI of 40 and over. It also accepts referrals for adults with a BMI between 35 and 40, if they have other co-morbidities such as diabetes or hypertension.

  • The specialist service is for individuals with a BMI over 40 or a BMI 35+ with significant co-morbidities and generally lasts for a 12 month period.
  • If you are thinking about bariatric surgery, this in an important part of the required preparation. Your GP practice will need to refer you into this service. You will need to have undertaken either a Tier 2 programme or the National Diabetes Prevention programme before you can access the specialist weight management programme.

  • Anyone aged 18+ with a BMI of 39.9 and over.
  • We can accommodate those who meet this criteria, who may also have learning difficulties/disabilities, physical or mental health conditions, and those who are pregnant.
  • If you wish to pursue bariatric surgery and have a BMI 39.9 and over, surgery may be pursued upon completion of the weight management intervention providing NICE guidance criteria has been met.

National

The NHS Digital Weight Management Programme - NHS England

The NHS Digital Weight Management Programme supports adults living with obesity who also have a diagnosis of diabetes, hypertension or both, to manage their weight and improve their health.

It is a 12-week online behavioural and lifestyle programme that people can access via a smartphone or computer with internet access

NHS Diabetes Prevention Programme (NHS DPP) - NHS England

The Healthier You NHS Diabetes Prevention Programme, also known as the Healthier You programme, identifies people at risk of developing type 2 diabetes and refers them onto a nine-month, evidence-based lifestyle change programme.

The Healthier You programme is available both as a face-to-face group service and as a digital service. When referred into the programme, people are free to choose between the two.

NHS Type 2 Diabetes Path to Remission Programme - NHS England

The NHS Type 2 Diabetes Path to Remission Programme is a joint initiative between NHS England and Diabetes UK available to eligible people in England.

This programme provides a low calorie, total diet replacement treatment for people who are living with type 2 diabetes and obesity or overweight.
